care plan template

Does any one have a care plan template or format that I can use as a guide.I have been searching on the net and can't seem to find what I'm looking for. I have a book to use as a guide for writing it out but not sure how to format the info.

here you go: careplan template. remember to use legal size paper. :) you might want to check out the student forums-- you'll get more clinical information there.
