Hey everyone. So I really suck at care plans. I am currently writing one for hemophilia. However my care plans for school now make me write another box out for evaluation . 1) I dont know what a box out is

2) I dont do your homework for you

3) Tell us what you think first. An RN prepares a nursing plan of care in the same way a doctor prepares a medical plan of care, based on assessment data used to make a diagnosis. What assessment data do you have on this patient or if its a pretend patient what assessment data would you have? What nursing diagnoses does that lead you to, what diagnoses do you make?

4) If theyre asking you for how to evaluate each intervention (since the whole plan of care is already right there for you INCLUDING EVALUATIONS AND OUTCOMES), THINK. How would you know that an intervention was working/effective?
