Care Plan help

I wonder if anyone out there can help me? It is so ironic that at 6am in the morning I, a student nurse is up working on care plans. Even more ironic is that I am not the only person on this forum!!

I am working on the very difficult care plan

The child is 6 years old newly diagnosed diabetic. The mother, father and aunt are the caregivers and have just leaned to inject insulin after monitoring the fingerstick. There are allegations of abuse, they are questioning the mother as to why she did not bring the child to the hospital sooner, after all, she did have at least one of the symptoms frequent urination.

I have to write a careplan with 3 diagnoses, short term and long term goals, nursing interventions and rationale. Anyone have any ideas??

Listen! At 6am I was over at Ralph's, a Supermarket chain out here in the west, buying cat food. It was that or get pawed to death at feeding time.

As I am sure you are aware, there is a lot of teaching with these patients. So, most of the interventions in your care plan will focus on education of the caregivers. Your short and long term goals will also reflect that. This should get you started and thinking:

Ineffective Health Maintenance R/T knowledge deficit in regard to the recognition and care of diabetes AEB inability of parent to take responsibility to observe and report patient's symptoms (and etc.)

Ineffective Family Management of Therapeutic regimen R/T complexity of therapeutic regimen AEB mother's lack of attention to the patient's symptoms

Risk for Injury: hypoglycemia R/T failure to consume enough calories

Risk for Injury: hyperglycemia R/T failure to take insulin properly

Risk for Powerlessness R/T age of patient and chronic illness

Risk for Trauma R/T inadequate precautions or the cognitive or emotional difficulties of the caregivers
