care plan help IDDM

here is the skinny. I have to make a care plan for a 65 y/o female with IDDM, and Alzheimer's. The problem is in the nursing Dx, it has to relate to the current dose of insulin becoming insufficient, she is becoming resistant to the current dose. Can someone help me out with this one?

here is the skinny on care plans. . .a care plan is an identification of a person's nursing problems. iddm and alzheimer's disease are medical diagnoses. insulin is a medical treatment ordered by her physician. that she is becoming resistant to her current dosage is a side effect of the insulin she is receiving or a problem connected to her iddm.

to diagnose requires assessing the patient's individual responses to her medical diseases and medical treatments. assessment consists of extracting abnormal individual data after performing:

  • a health history (review of systems)
  • a physical exam
  • assessment of the patient's adls (at minimum: bathing, dressing, mobility, eating, toileting, and grooming)
  • reviewing the pathophysiology, signs and symptoms and complications of their medical condition
  • reviewing the signs, symptoms and side effects of the medications they are taking

all nursing diagnoses are dependent upon the patient having signs and symptoms that match at least one or more of the defining characteristics of the nursing diagnosis that you decide upon using. it requires that you consult a nursing diagnosis reference. there are a number of ways to acquire this information.

  • your instructors might have given it to you.
  • you can purchase it directly from nanda.
  • many authors of care plan and nursing diagnosis books include the nanda nursing diagnosis information. this information will usually be found immediately below the title of a nursing diagnosis.
  • the nanda taxonomy and a medical disease cross reference is in the appendix of both taber's cyclopedic medical dictionary and mosby's medical, nursing, & allied health dictionary
  • there are two websites that have information for about 80 of the most commonly used nursing diagnoses that you can access for free:
