I've worked with the 2 LIFE Centers in Philly---goal is to help people age healthfully and remain in own home. Medical Director of program becomes PCP and they manage & pay for all care from hospitalization, pay for meds, eyeglasses, at home care along with encouraging weekly visits to senior center to improve socialization..... Center also provides PT/OT, podiatrist, hairdresser/barber .....can even do laundry at ours!
