I have no idea what nursing diagnosis to give a client with blood in their stool! None of the assessment I did on the patient gave any clues. She was pretty much normal besides that. Please help me!

Bowel eliminiation alteration, gastrointestinal alteration, those were the two that jumped out at me looking at the NANDA list.
I know when I have had my personal bouts with ulcerative colitis, I would have diarrhea with the bloody stools. So you could use that diagnosis Diarrhea if they are having it with the bloody stools.
