Blood in stool care plan

I have no idea what nursing diagnosis to give a client with blood in their stool! None of the assessment I did on the patient gave any clues. She was pretty much normal besides that. Please help me!:o

Bowel eliminiation alteration, gastrointestinal alteration, those were the two that jumped out at me looking at the NANDA list.

I know when I have had my personal bouts with ulcerative colitis, I would have diarrhea with the bloody stools. So you could use that diagnosis Diarrhea if they are having it with the bloody stools.
