Can someone please clarify what type of documents are needed on the SOC visit.(for home Health)I have had several people tell me different ones. What general but most important documents are needed for example like the patient service agreement, medicine schedule, etc. Thanks

soc consists of:

forms to be signed or filled out by patient/cg:

consent form

medicare payor screening form

emergency plan form

route sheet (end of visit)

Sn paperwork:

SOC OASIS

md order - to state frequency and orders

medication list

abn form (completed upon dc)

other paperwork to be left at patient home:

emergency protocol for patient handout

basic home safety precautions handout

vital signs sheet

sign in sheet

Not all agencies use the same documents nor do all patients within one agency necessarily need the same documents. Medicare patients, in general, need a lot more than private insurance patients.

I do pediatric home visits. The parents sign a service agreement and the children get a comprehensive pediatric assessment. Paperwork left in the home includes things like privacy practices, patient rights/responsibilities and the agency's contact information. I almost never fill out MSP questionnaires or ABNs because children do not have Medicare (except in very rare instances, like kidney failure). We do not leave vital signs sheets nor sign in sheets in patients' homes.
