ADHD Guidlines

Hi all i have been searching everywhere for this info, How often should the pt with ADD or ADHD have a written re evaluation done the assessment where both the parents and teacher fill out the form. I am a new PNP and dont remember ever covering this I know we do a initial evaluation, but the MA in our office said that the pediatricians office she used to work for did this every 3 months, but then again this same person told me that I should do the Lancaster questionairre to assess for depression in new mothers at the 6 week well child check any info is appreciated thanks

thanks where did you find the guideline I even ask my md and she was not sure either, as it is I have not been doing the initial eval on the new pt's in the office, when I get the records from the old provider there is only one eval the one that was done initially and some of these are 5 years old gulp
