What do you do?

I have a pt. that gets his meds from the VA. He insists on filling his own mediset. I saw him on Monday he told me he filled his mediset and that he needs more Coreg and Gabapentin. I called the VA to get these filled and sent to him. I got a call from his VA nurse on Thurs she said the pt told the social worker he is out of coreg for the last 2 days. I told her I called them in on monday she checked and they did not get shipped out till Thur and it was shipped from NC and he lives in PA. I called his PCP and she called in a bridge script but he had to pick them up. I called pt he said he would get his neighbor to get them and he would take care of it. Well I called him Friday night and he did not get them. The VA pharmacy is not open on the weekend. I went to visit him on Sunday his BP was 98/62 and he felt fine. Neighbor will pick up script early Monday AM. I very worried about him but not sure what else I could have done. My companies policy states we are not allowed to go pick up medications but my manager told me that some still do it. But the pharmacy is not open and I thought he had the mediset filled till end of week and the new bottle would be in by then. When you have a pt that lives alone and is slowly declining what can you do? I call him everyday and see him 3 times a week which is the max visits I am authorized to do. He is a MC pt.

Any suggestions.
