Can anyone help me with titration problems??? Do you know of any website that can also. I really want to excel it titration but I get confused towards the end of the step, my mind goes blank.
Here is a problem sample; Give lidocaine 30 mcg/kg/min to a child weighing 55 lbs. the drug available is 120 mg/100 ml of fluid. How many mcg/min will be given? Mcg/hr? and what will be the infusion pump rate?
Another one I blank on is not a titration but what I call "What" question. Here is a example. Medication "X" 40mg IM. the patient's medication tray contains two single does vial of 15 mg and one signle dose vial of 30 mg. what vial or combination of vials are needed to be used for this initial dose. Also identify the vial(s) that is needed and the number of mls' needed from each vial(s). than indicate how much mls' in the syringe? - "What?":madface:

For your first question the answer I came up with was 45 mg/hr. The pump setting would be 37.5 ml/hr. I took the child's wt, converted to kg., (25kg), multiplied that by 30mcg ( that's 750 )then I multiplied THAT by 60 (60 minutes in an hour). 45000 mcg per hour or 45 mg per hour. Then I got to the medication on hand - The way I did it was divide 120 by 100 for 1.2 mg/ml. Then I divided 45 mg per hour by 1.2 to get the rate of 37.5 ml/hr for the pump. (I never could do the ratio thing, but if I have a calculator, I usually get the same answer as my ratio competent co workers. ) As for your second question, there really is not enough information given. Single dose vials come in differing concentrations. If the concentrations are 30mg/ml and 15mg/ml , I would draw up the 30mg/ml and 10mg from the 15mg/ml vial, giving a volume of 1.33 ml. If the vials are 15mg/ml and 30mg in 2ml, then I would draw up both 15mg/ml doses with 0.7 ml from the 30mg/ 2ml vial for a volume of 2.7 ml, since this is an IM injection and you are supposed to keep injection amounts at or below 3 ml.
