Syringe driver

Hi I am absolutely mortified, I was off for a few days and a lady was in a driver , I came back found administrative chart and look at label on driver I could not find prescription chart I asked the night nurse about script he said the day nurse said paperwork has changed I put driver up with the information I had palliative care nurse saw patient so did community nurse to check on her PRN meds it came to light not enough water in syringe I feel so ashamed
