Some questions about NCLEX application

I have two questions regarding NCLEX application in California.

1. One of the requirements in applying for NCLEX exam is that I must provide a copy of my diploma or license stating that I can practice my profession. Is it ok to send a photocopy of my license card or it has to be scanned and printed?

2. The NCLEX application ask me to paste a 2x2 passport type photo. What does it mean? What background color? Do I have to look formal in the photo?

I don't know if I posted this thread in a correct place. I just think this was the right one for my inquiries about NCLEX application.

Guys, I have another question about the application.

My license here in the Philippines will be issued in November 26, 2008. It is quite long and I want to pass my application for NCLEX as soon as possible. The application requires international graduates to provide a copy of their diploma or license card. If they do not hold any license, the applicant must provide a written explanation stating the reason why he/she doesn't hold a license in his/her country.

My question is:

Is it ok to pass my diploma and I will write a written explanation stating that I will be able to get hold of my license in November 26, 2008? After I got my license I will forward a photocopy of it as soon as possible. Do you think it will work? Does will cause any delay to my application process?
