  2. Bottom Line imho: You will end up treating some drug seekers with narcs to treat people truly in pain. Big deal. We all know what they are like, and how slimy it can feel. The only ones I refuse are the obvious very manipulative types that I have personally treated in the past. Even then, an open mind is important. But in the end, I sleep better knowing no one left my care in pain when it was in my power to alleviate that pain.

  5. 1. please give a full report of what you found at the scene, or what the nursing homes concern was in a patient who can't or is poorly able to give a history. You are our eyes and ears!!! Context is everything! Hang out until you can say hello to the doc, unless the case is bogus. 2. Please quickly run down the interventions you did en route. 3. Ask the family the code status if you have any chance to, and bring in DNR papers and meds! Thanks for all you do, you ARE appreciated!

  7. I was taught in residency never to give phenergan to a child less than 3. I believe there are valid reasons: 1. It makes the child too dorwsy to orally rehydrate/maintain hydration... 2. How can you tell if the child is lethargic if they are drugged from phenergan? This is why I order so much fluids on vomiting children despite the eeevil glares I occasionally get from our nursing staff:p I agree that dehydration killed this child, not the phenergan. I am terribly sorry for your friend's loss.
