I'm not sure what you're asking here.
What you've written is a statement, a description OF the patient that has come to the hospital.
Is there more to the question?

I think I understand what the OP is asking. The question is stating that a 40 year old client is admitted to the hospital and he happens to be on digoxin. So the question is which of the following statements (the OP doesn't state which statements) made by the client is important for the nurse to follow up. The question is asking which of the following statements made by the client is WRONG- therefore the nurse needs to follow up and do teaching if that's the case. Hope this made sense.
