pivas score

Hi

I need some help here..

I am really confused...

I know what PIVAS score is and how to check the site, but what makes me feel lost is the documentation.

For example, when a patient has a IV cannula in situ and I just gave him NaCl IV fluid. Where do I document the findings? I found a PIVC chart, but I think this is recorded only when the cannula is inserted or removed. So the only place left is the patient progress note.. I am not sure..

Can anybody help me with this..?
