Phnat Tool

Hi everyone. Any nurses or nursing students use the Public Health Nursing Assessment Tool (PHNAT)? I have a capstone project I am working on using this assessment, and I am having a hard time. My community is Lee County Florida and my population is patient's with Chlamydia. My question, does anyone have any links of examples or helpful resources? Any information would be helpful. The stuff, I am finding on the internet is scarce.
