I need help to make sure I am on the right track with may pediatric math. The problem states: Digoxin elixir 85mcg p.o. q12 orderd for a child weighing 35lbs. You have Digoxin elixir 50mcg/ml. Recommended dose range is 0.01-0.02mg/kg/day in divided doses q12h. a).child wt kg b).safe dose range c). is the order SAFE ? d). if yes how many much will be given? if some could help me with steps please!!!!!!!!!! I have a test in 2 days with only 2 trys thank you.
Step I Convert wtinlb to wtkg then multiply to max rec daily dosage:
35lbs/2.2 = 15.9kg (where 2.2 is the conv factor) then
15.9kg x 0.02 mg = 0.318 mg/day = the daily max rec dosage
Step II Conv mg to mcg, divide dosage and figure out if within dosage range
0.318mgx1000 = 318mcg (where 1000 is conv factor)
318mcg is the max dosage for the part wt/day, then:
85mcg q 12h, to be give twice so mult by 2
85mcg x 2 = 170 mcg/day ordered dosage, which falls within range
Step III Figure out how many cc per dosage needed
50mcg/cc x 85 mcg = 1.7 cc

Okay..here goes:
Child weighs 35 lbs divide by 2.2 = 15.9 kg (1 lb = 2.2 kg)
safe dose range:
0.01 mg times 15.9 kg = 0.159 mg (0.159 daily divide by 2 = 0.0795 mg)
0.02 mg times 15.9 kg = 0.318 mg (0.318 daily divide by 2 = 0.159 mg)
ordered does is 85 mcg q 12 hrs (85 mcg divide by 1 mg/1000 mcg) =
0.085 mg q 12 hrs is the ordered dose
so, 0.085 mg times 2 (q 12 hrs) = 0.17 mg daily dose
so the dose is safe: 0.17 mg falls between 0.159 mg and 0.318 mg daily
or each 12 hr dose ( 0.085 falls between 0.0795 mg and 0.159 mg)
