DOES ANYONE KNOW OF ANY PT FLOW SHEETS AVAILABLE ONLINE OR ANY IDEAS DFOR SAME . WHAT DO YOU USE?

My unit includes oncology pts. I may be able to help, we have several flowsheets. Can you be a little more specific in what you are looking for... Thanks
THIS IS FOR AN OUT PT. AMBULATORY CARE ONCOLOGY CLINIC. WE NEED A GOOD FORM THAT CAN BE USED FOR STORING INFORMATION RE: CYCLES , LAB WORK,ECOG PERFORMANCE STATS,MEDS ALLERGIES , BSA--- ETC....tHIS WOULD ALSO BE SENT TO MD'S OFFICE FOR THEIR RECORDS.
Thanks that helps. We have a brand new cancer center at our hospital. I do not know what types of charting forms or flowsheets that they use, but I will certainly ask for you.

The words got moved around a bit when posted
Basically you have the 4 things with blanks beside them in the top left corner and there is a blank on the top right corner for the pts addressograph.
Then everything is on the left side of the page with 4 columns to allow for 4 days. There is a column on the right for writing nursing notes with initials and signature spaces at the bottom of that.
