Nurse Notes HELP!!

I'm a new fresh nurse on training. English is not my first language, and I am having difficulties to write notes such as admission or discharge notes. 

Someone could help me by giving me ideas to how start,  on what I need to focus on (assessment, interventions, etc) and how ends.

In my facility, notes are usually  write when something uncommon occurs to avoid double charting, and short notes for asmission and discharge.

 

Please, help me.

 

I'm on my 5th day of training, I feel I know nothing, what makes me feel frustrated and anxious.
