I have read several different answers to this question. Is it correct that the fundas is at umbilical height after placenta is delivered, 1 inch above 12 hrs after, and 0 - 1 on day two? This is the qustion from me studying throught ATI. I get conflicting information through Lippincott.

What is the correct way for NCLEX?

A nurse is performing a fundal assessment on the client's second postpartum day. Which of the following should the nurse expect if the client is experiencing normal involution?

The fundus will be one centimeter above the umbilicus.

The fundus will be at the level of the umbilicus.

The fundus will be one centimeter below the umbilicus.

The fundus will be two centimeters below the umbilicus.

1-2 hrs after birth the fundus is midline bet umbilicus and symphysis pubis

6-12 hrs after- at the level of the umbilicus

day 1- 1 cm or 1 fingerbreadth below the umbilicus (U-1)

day 2- 2 cms or 2 fingerbreadths below the umbilicus (U-2)

day 3- 3 cms or 3 fingerbreadths below the umbilicus (U-3)

This rate continues downward until the uterus is not palpable

approximately day 9 or 10.

Ok, so...to the question:

That's two days which is 48 hours PP.

I'll go with 1 'below'.

... because the fundus is 'at the level' right after birth.

12 hours PP, it's 1 fingerbreadth 'above'. Then it starts dropping 1 'fingerbreadth' below, every 24 hours.

It can't be 'at the level' because it's 48 hrs post.

I didn't pick 2 because... I don't know. LOL

I kind of saw the 12 hours as being included in the initial 24 PP? Fundus begins at the level.

It moves 'above' during the first 12, then it's 'at the level' when the 24 hr PP mark hits.

That's 1 day.

Then, it descends 1 cm every 24 hr pp after which brings it below the umbilicus.

That's 2 days.

So... 1 cm 'below'?

I'm guessing. Could be wrong. I don't remember the exact particulars.

I just know that it goes up in the first 12 hrs PP and is midline at day 6.

As I understood it, it's fine that it pops above the umbilicus initially. So long as it continues to descend within 24 hrs (or every 24 hours), that fundus isnt tilting to the left/right, feeling mushy (boggy) and something's going wrong with the woman's 'discharge' (forgot what it was called)?
