I broke my wrist and was selected to test the day I went to see the orthopedic doctor from urgent care. I submitted my test as I always do and was going to stop on my way to doctor all I’m one trip. I discovered after arriving there the place was closed bc the lady had a stroke. I had Affinity send it to a different location but I was tied up getting scans and casting of my arm that I didn’t make it before they closed. I notified Affinity and my case manager but never heard back from case manager. Then 2 days later I missed a check in as I’m in severe pain waiting for surgery in 2 more days that it slipped my mind. I’m very upset bc I've been doing this for 5 years and have 3 months left and feel like I just screwed up big time. Has anyone else missed a test? I notified my case manager again (5x’s total) with no word back. 

Did you miss a test and a check in or just a check in? Check in probably isn't that big of a deal, but missing a test is like failing a test. Go to an urgent care or ER if the place is closed.

I’m sorry you are going through this with 3 months to go. May I suggest that you send your CM an email explaining what happened and include as much documentation as you can, d/c paper work, receipt from your doctor’s visit, etc. If you haven’t miss any test or check ins in the past they may give you a break and only ask for extra tests. Please keep us posted and good luck with your surgery.

I definitely have all the documentation of my doctor visit and surgery. I can’t believe I’m the only person to miss a test for whatever reason. I still haven’t heard back from my case manager and I’ve sent several emails. I just want to prepare myself for what’s to come. As far as them considering it a failed test, I’ve been prescribed pain med which I have emailed RX’s to case Mgr and affinity so it’s not like I’m hiding anything.
