MAR error?

I don’t know if this is the right area to post this.  I’m probably just being paranoid but was curious if it is possible in any charting system you’re familiar with for someone with high enough access to make a medication order completely disappear from the MAR.  Not discontinue, completely remove the order for that medication.

Did you look under discontinued meds?
