Hello, Mine is 3 pages long and I am currently downsizing to 1 sheet. The problem is everyone started asking things to be added for their convenience in auditing and grew to 3 pages. I am looking for other institutions and what do they have on their reports. Mine goes to ELT and all leadership but I am not familiar what others report out to leadership in safety huddles. I can give you a copy when mine is finalized. I have everything from # of 1:1's (sitter needs), MRN#'s of people in restraints and on isolation, staffing and where they were floated to, census of each floor, staffing per floor, facilities issues, AMA's, OR cases during the night w/MRN#, if AOD was called, falls, codes and rapids w/MRN#'s, etc. You can see how this becomes tedious for house supervisors. Anyone who can share if they send a report to leadership and if they do, what is on it, would be helpful.
