Help-Radiology informatics question

Hi, I am hoping someone can help me in my search, or even better, share your successful practice in your own hospital. We have had several "errors" where radiology notes an incidental finding on radiology results and for some reason or another it is not acknowledged at discharge, passed on to the patient's primary care, or even disclosed to the patient. I have found that, throughout my healthcare system, each facilities contract with their radiology group differs and that process's of discrepancy reporting and incidental reporting varies. I am trying to standardize the process throughout. Our IT imaging personnel have shared that other facilities have better ways to ensure that findings are passed on to patient's and their providers-but unclear of the specifics. Where is a good place to start looking for the best recognized practices for disclosing imaging results-utilizing Epic if possible? Anyone want to share their success story of implementing a new process? Thank you for ANY feedback!
