Hello all. I took care of my first client the other day :) Good stuff. Different in some ways than I thought it would be, just like I thought it would be in others.
Anyway, my client was a 58 yr female admitted from the ER with a diagnosis of R/O TIA. Her main complaints were HA and dizziness. She did display some orthostatic signs while I took care of her, and I noted her vitals lying down, sitting up and standing. (The difference in B/P sitting up and standing was sitting: 125/68 standing: 99/70)
I have to decide a nursing diaganois....
I have come up with : Impaired physical mobility, risk for injury, risk for activity intolerance, altered tissue perfusion (orthostatic)
Any suggestions? should I re-think any of them? Add to them? I did consider altered thought process, but I didn't find her to have any problems with thought process or confusion despite the possible TIA.
