Does anyone have a sample of a Kardex for a telemetry, ICU or Med Surg area?

Hello, I am a manager of a telemetry unit and am desperately looking for samples of Kardex's or Patient Order Summary sheets. If anyone can help i would really appreciate it. My e-mail address is [email protected]
