im in desperate need of help!! ive only done care plans for my patients but this semester my professor wants a concept map. i kinda sorta have an idea of what im doing..i think. can sumone let me kno if this is right??
-my center box has the admit dx of gangrene.
-a box with my assessment of the pt (his vitals, r lower extremity has erythema, warm to touch, pedal pulse heard with doppler, first/third toe black/yellow in color, oderous)
-a seperate box with med hx (no medical hx, smokes 2.5 ppd x 40 yrs, etoh)
-another box for labs (WBC- 18.3 H, PLT- 408 H, APTT- 44.2 H, PT- 13.7 H, INR- 1.21 H,Glucose: 102 H Urinalysis:Protein- 25, Ketones- trace, Urobilinogen- 4)
-another box for the procedure he had done (right sfa angioplasty and stent placement)
-another box with all his meds (Zosyn 3.375G IVPB q 8hr (infuse over 4hr with D5W),Folic Acid 1mg PO x1 daily,Maalox Plus 30mL PO q 6hr, Nicotine Patch 21mg PT24 TD x1 daily, Plavix 75mg PO x1 daily, Prednisone 40mg PO x1 daily, Prilosec 20mg PO x1 daily
Thiamine (B-1) HCL 100mg PO x1 daily, Ambien 5mg PO HS PRN, Ativan 2mg PRN, Morphine Sulfate 2mg/0.2mL SC q 3hr PRN
Percolone 10mg PO q 4hr PRN, Senokot 2 Tabs PO HS PRN,Tylenol 650mg PO 6hr PRN,
Heparin/0.45%NaCl 25,000units/500mL IV (D/C)
*so im wondering with his meds do i need to list every med?? or just the meds for his infection and pain
*and with his labs do i need to write in the box wit the labs y his levels r high? or does drawing the line to the center box of dx of gangrene explain y the levels r high.
*and do i add a box next to the one with his hx with smoking and put in the new box "Teaching:" smoke cessation and also put interventions in that box about giving his nicotine patch as prescribed??
thnx for ur help. i kno im makin this harder than it has to be, but my professor isnt very specific when she explains this stuff. she told us "look it up on the web" but the examples on the web are confusing me
