Communication Handoffs

Does anyone have a policy out there on "Communication Handoffs" This is a new Joint Commission Safety Goal and I'm wondering how other facilities are handling this. We have developed a "Communication Sheet" that we use when we take report on a patient coming back from PACU or a new patient being admitted from ED or another facility which the staff really like...now we are needing to write the policy on this. What is everyone else doing?

Check the jcaho web site at http://www.jcaho.org. Look under the Patient Safety tab and then National Patient Safety Goals (NPSG) FAQs. Also, a good reference is the Joint Commission 2006 National Patient Safety Goals Implementation Expectations at the same site.

We have a policy, but it's much too large to send in a reply.
