Care Plan Help Please!!!

i am in nursing school and i am writing my first care plan with an actual patient. i have two nursing diagnosis based off my priority medical dx of alzheimer's. they are imbalanced nutrition less than body requirements and fluid volume deficiet. i am having a hard time with my r/t and evidenced by because i think i am mixing them up. i need help please!!!

r/t inability to ingest food because of loss of appetite due to alzheimer's disease as evidenced by weight loss, lack of interest in food, high bun level

i was wondering if that looks right or makes any sense. this careplan is 80 percent of my grade and i just want to make sure i am going in the right direction
