CA BRN Deficiency - Third Party

Hello! 
I sent my application in Breeze for CA - RN. In my portal I am receiving the deficiency pop up with "Third Party Authorization Missing". As I was reading the instructions, it seems to be a form or something to submit to release this information. I cannot find a specific form, nor can I get a hold of CA BRN on the phone... Did anyone here have the same problem and got it resolved? Or did it delay your application? Thanks for the help
