Hi everyone,
I have the most trouble when it comes to priority (I can never get the NCLEX's test makers' way of thinking when it comes to the timing of preforming a task. We are drilled ADPIE and prioritize ABCs always, but there are many questions that do not follow this and the rationale is often vague and very debatable). If anyone did well on their NCLEX or school exams with these priority questions, what resources do you find the most useful? I don't want to become too scattered and use too many. Much thanks!
Example:
"A nurse is suctioning a patient with a tracheostomy. The patient suddenly develops a heart rate of 38 bpm. What is the priority nursing action?
A. Administer atropine
B. Stop suctioning and assess the patient
C. Begin CPR
D. Increase oxygen flow rate
~~~
Correct answer: B
Rationale:
Suctioning stimulates the vagus nerve, which can cause bradycardia. First action is to remove the stimulus.”
And if you thought atropine (I did), then this:
"Why NOT atropine first?
Because NCLEX wants least invasive → escalate:
Remove cause (stop suctioning)
Reoxygenate / recover
Then atropine if bradycardia persists
If the question said:
HR 38 after suctioning stopped
OR patient symptomatic (LOC change, hypotension)”
(To be transparent, this is an example from ChatGPT, but I find that my GPT barely have errors now and it also explains the rationales to other answers from Kaplan or Saunders well, but I just don't follow this logic flow)
