Assessment writing

Are there any links to provide help with writing (not using a form) an initial assessment? I've seen many checklist forms for completing an assessment, but what about links for students who are required to write an assessment without the use of a checklist form. Specifically I'm looking for proper descriptive terms used by nurses in describing appearance, grooming, thought processes, speech, pulse characteristics, heart sounds, venous patterns, hair distribution/color, etc.
