AIO Charting

I really need help with AIO charting. I know that each problem is separate, but do I chart the interventions, i.e. dressing changes, etc? I guess the outcome is really where the confusion sets in. Is the outcome considered a short term goal or what? Any suggestions will be greatly appreciated. TIA. :confused:

I think maybe I should explain a little more about my post. Maybe it will be easier to give me some suggestions. The patient has multiple integumentary problems, i.e. bruising, skin tears, pressure ulcer Stage II & stasis ulcer. If I am able to get some info on that system, I should be able to figure out the rest. Thanks again.

What is AIO charting? I googled it and didn't see anything.

Yes, that is what it is. It is suppose to be basically the same as PIE or DAR. This is really getting frustrating.
