Kudos Berube!!Most of the docs in our area are this way. The only thing they usually order is the antibiotic!

Thanks Berube!!Callicotter--you always have to get an order for any kind of wound care. Most docs just want you to suggest to them what to use:coollook:. I feel like I need to be wound care certified to work in home health. We do have a wound nurse in another office that I get suggestions from. But it doesnt hurt to get other opinions. Thanks for the help!

I am going to try the 20g tomorrow and with different position changes. I had left a message with the surgeons nurse and she later called me back stating the doc said there was nothing he could do other than change the port and we could just use it for meds. It flushes easily, but makes me uncomfortable without the blood return. If I dont get it tomorrow, would you suggest to the doc about TPA????

He did. Doc gave him antibiotics and referral to home health to eval and treat.

I have a patient that is a monoplegic and had fx his femur one month ago. Dr put on compression bandage (left it on for one month), pt took it off the other day and had alot of swelling in his left foot, open area on top of foot, and entire heel is a stage II. Alot of clear serous fluid from wound. Patient is already on a diuretic. I put abd pads and covered it with kerlex and told pt to elevate foot etc. What would be good to put on the large wound on the heel?? This is patients bad leg, no feeling or movement in leg. Thanks for any suggestions.

I thought about the positioning (after I left). Maybe I will try to go back tomorrow and try a different position. Thanks!!

Again, thank everyone for the great advices that is always given!!I have a patient that had a port placed 4 months ago. The surgeon ordered it to be flushed q 2 months with sal/hep (i thought it was supp q monthly). The port was flushed 2 months ago by an RN who is no longer with the company. When I went to flush the port yesterday, there was no blood return even with 2 attemps. Port flushes easily, pt states no pain. I called the surgeon for further instructions but have not recieved a call back yet. PT is a quad but has some movement in arms and upper chest (not sure if this has anything to do with it). Help:eek:

I am a home health nurse doing an antibiotic infusion through a port. I have not done one in a while. After the port is accesed do you put the biopatch under the huber needle then cover with tegaderm? Change q 7 days??

I have drawn many labs from picc lines when I was working in the hospital. Most of them were the power picc lines. I have a home health patient that has a picc line that has no clamps on it and the end caps are horizontal, unlike the power piccs. It flushes easily but does not have any blood return. There are orders for a lab draw. HELP.

Sorry Cherubhipster...it is so:crying2:. But after a year, u can move on to something better!!! Whow knows, there are some that actually like working in the hospital. I was the one who was throwing up before and after my shift!!!

I am a home health nurse and did an ABT infusion on a client this afternoon. He has a double lumen picc line. The order is the same for the infusion like using sash,and dsg change q 7 days or as needed. There are no orders to flush the other lumen that I can see and we always flush them with ns then with hep QD in the hospital????

I have started with a new home health company and we also use the same monitors. I am still learning how to use them.

Reading this reminds me why I left the hospital setting. I chickend out and could not do it anymore. I commend you for sticking with it.:bowingpur:bow:
