I am a consultant in LTC. Yesterday, I watched staff swoop in and do the right thing for a patient with widely metastasized cancer. After her physician of record d/c'd her percocet and had her on Tylenol. In a flash she was transferred to a different floor where a different physician provides services. I'll get out of the way and let you people do what you know how to do.

It is ingrained in us to serve as advocates for our patients, to speak up for those who either cannot or will not speak up for themselves. I agree with ruby_jane. My last concern with a patient with metastatic cancer would be addition to narcotics. Comfort measures are about all that you can reasonably provide in that situation. NSAIDs like Tylenol won't touch pain of that degree.
