I already know that Hospitals do not sponsor RN's for Green Cards or H1-B Working Visa. So do facilities like Nursing homes/skilled nursing facilities/Long term care and/or Home care Agencies sponsor?

The unavailability of sponsorship is not exclusive to hospitals. So nursing homes and ALL other health care facility are not exempted. For RN's, the only way you can penetrate the market is if you have an NP/CNL/CNS qualification AND the job position requires one of these qualifications. Alternatively, if you are a specialist RN, you probably can try the H1B route. But even with full qualifications, you have to understand the employment situation of the US and how will this affect your chances.
I am a new grad with OPT. So what are my chances of gaining employment anywhere? I was denied employment because the hospital did not sponsor when I asked them during the interview. They said they were very impressed with me except for the fact that they do not sponsor so they did not hire me. Plus I am a new grad which makes the job market even harder. So what should I do now? I don't know where to go with my OPT

You need to find employer quickly as I understand OPT is only valid for certain amount of days without a employer after that you will need to maintain student status. You really have to spend time getting employers interested in you. Even if you find a sponsor you can't stay and work whilst waiting for immigrant visa to come through. H1b unless hospital is not affected by cap can not be applied for until next April with an Oct start
