I have recently been informed that I will be working as the PCMH Patient Care Manager at my place of employment. I work for an organization that encompases 6 clinics in a rural area in the upper midwest, not far from the Canada border. The clinic that I work out of has 5 family practice providers. I have worked at this clinic for several years as the Health Educator and Anticoagulation Nurse. Because we are now going to be implementing changes in regards to the PCMH model, it was decided by the management that my responsibilities would change from what I have been doing for many years to the Patient Care Manager. At least initially, I will be the only PCM working with our patients with CHF, HTN, DM, etc. I have been an RN for nearly 30 years with a vast variety of experience, including case management years ago. Because the PCMH concept is new to our organization and largely to our area in general, I have no idea what the pay range would be for a nurse with my experience in this position of Patient Care Manager. Would anyone who currently works in this type of position or is aware of the pay for this type of position, be willing to share that information with me? I am planning to request a pay raise due to these new responsibilities, but really have no idea what a "fair" pay would be.

I work in a system that uses the PCMH model. I am a patient navigator and work closely with clinic care managers. The clinic care managers pay range is between 45k-65k. My role range is 55k-75k. PCMH seems like a new concept to some areas but it is really just getting back to basics!
