I was wondering what other institutions policies are regarding transporting pt admitted to telemetry beds from the emergency room. Our hospital policy states that these patients (not step down or icu) require either a rn or a emt/iv tech with a monitor and defibrillator during transfers.

Our hospital requires transport to tele and ICU with RN, monitor, defib as well. In fact, in all my years in nursing it has been that way.

Tele: Paramedic with monitor and defibrillator
ICU's: RN with monitor and defibrillator
Techs can transport to M/S but any infusing antibiotic the patient has must have been up for at least 15 minutes to assure no allergic reaction in the elevator.

At my current facility, an RN must transfer the patient to the tele floor with cardiac monitor in use (the monitors we use for transport do not have defib capability.) In fact, I am a new grad and only recently took ACLS, and was told we could not transport to tele by ourselves until we passed ACLS. We do not have EMTs working in our ER so that's not an issue.
At the facility where I worked as a tech, the RN had to transfer the patient to tele. But, there was an option on the admit/transfer sheet that the admitting MD could check off which stated something like, suspend monitoring for transport. If this option was checked, then one of the techs could transport the patient.

if admit to telemetry, cardiac monitors were a must along with a licensed person, however, it was always a fight when the OR came to pick up patients from ICU....for some reason, they thought monitors were optional, perhaps having a CRNA or anesthesialogist present made the patient stable enough to transport without a monitor???...lol

RN with monitor. No defib or ACLS drugs.

From ED to tele bed, just a transporter and the pt gets put on remote tele monitor that they stay on once on the floor.
From ED to ICU, an RN plus portable monitor/defibrillator.

Our hospital is small, and the design is you walk by the ICU (code cart location) on the way to our telemetry unit. Somebody starts getting hinky, they just make a sharp right hand turn. RN transports with telemetry going the whole time.

Medical Surgical Floor: BLS tech
Cardiac/Tele Floor: ACLS RN + tech, patient on defib monitor
ICU: ACLS RN + tech, patient on defib monitor, along with a kit with ACLS drugs (epi, adenosine, etc)
