I've been out of Nursing now for about almost 5 years due to personal and medical problems. What do I need to do to get back in?

Do you have an active RN license at this time? If not, the very first step would be checking with your state board to find out what you need to do in order to reactivate - which may include taking a refresher course.

I just have to renew my license. I've also been advised to enter a monitoring program.

If you do need to enter a monitoring program, I'd advice checking out this forum: https://allnurses.com/nurses-recovery-c166/

I don't think working in psych again is out of the question, but depending on what kind of monitoring program/your specific stipulations, it could make things more challenging.
