Venting, being blamed for another home health aide's error

My newly diagnosed diabetic client was 'reminded' by my co-worker to take a second insulin injection, the verbal order from M.D. is for one injection per day. I contacted the R.N. to advise. Somehow she is blaming me for my co-worker's action. The M.D. For some reason refuses to write an order when pt is in the office. I attend the M.D. visits w/client and have requested the written order, the M.D. says he faxed the order to R.N. And gets mad at me for requsting an order he already 'took care of'. There is a visiting nurse who is supposed to manage the medication and get the orders from the M.D.

I'm more than p$&@ed that I am being scapegoated for this serious mistake.

I'm not a nurse, i havent gone beyond my CNA scope of practice and I didnt remind the pt to make a very serious medication error.

I don't understand either. Are you trying to say you were blamed for the client taking a second insulin shot because the prior HHA said to?

If this is the type of relationship you have with the nurses that manage these cases, maybe you should consider looking for a different agency. Meanwhile, I would write up a clear description of what occurred and send a copy to the agency and keep a copy for yourself. Just be ready in case this subject comes up for your job evaluation. And just be aware that updated orders often fail to get from the doctor through the agency to the people working on the case. It is usually the fault of the agency because they are supposed to make a copy of the signed order available to the nurse or mail it to the home.
