Has anyone received the correct preference form?
I opened the attached file from their email and it was just copy of email in image file.

I emailed them, but I do not know when they are going to respond.

Yes! I I received the form as well. Has anyone heard anything yet? I only received the email asking me to send the essay and teanscript. (I already uploaded them when I applied)

literally SAME LOL. I uploaded all required documents for Dallas and Frisco. One recruiter needed the transcript again and the other needed a preference form and resume again. So im not sure what the mix up was. I sent them day before yesterday and I  haven't heard anything back yet.

Same as well. I applied for Dallas location and received the email asking me to complete the preference form and send back. I haven't heard anything else yet either.

I applied on 1/26 and didn't get a reference form just like you. I emailed to the email that I supposed to send a preference form and they send me one. Finally I was able to send it yesterday...

Hi! Do you mind sharing the email that gave you the preference form? I applied on January 29 and have not received a preference form yet.

Not yet, but I'm really hoping to hear back from them. THR is looking more and more like my top choice.
