I'm currently in the program. We had 200+ applicants, I believe 50(?) interviewed and 20 in my cohort

Is anyone applying to this program from out of state? Do they consider applicants from other states? I'm in Texas!

yes. I live in Missouri and am in the program.

Do they require the GRE? I am trying to find the code to send it to, but not finding one.

They do not, but you can add it to supplemental info/documents.
