Tell me about your triage layout

For those of you with dedicated triage rooms, can you tell me about them?

Specifically, safety issues.  How does the layout take into account safety?  What is the proximity to the waiting room,. and what responsibility does the triage nurse have for the waiting room?

Are there any with doors open to the waiting room?

There are two exits for every room in the triage area. we have an alarm that can be worn, and alarms at the desks. If I'm triaging the secretary can hear whats being said, and sometimes security lurks just outside the door, if the patient is loud. The staff area is locked from the waiting area, and that has saved us a few times, but then the other patients are exposed until the police show up.
