I have a coworker who I often have to come and work behind.....she keeps changing the times on a pt's meds b/c she goes to dialysis 3 dys a wk. ....so on top of all the insulin, coumadin, and 5 bp meds she gets @ hs.....this nurse put the times very close together for another bp med thats ordered tid.....to me tid is 8 hrs apart ...she put it for 4p and 8p and 9 am....the 4 and 8 being my time. I thought it was way too much and too close together so I put the times back the way they were originally...well....how about this mess....I work the other day and i see that she had changed the times AGAIN...AND this time...she over did it.....she falsified the record......the 3rd shift nurse has bad handwriting....like me but way worse.....ok....i look and see her initial for this med only...@ 6 am were circled...so i look to see why........"refused meds" was written on the back with no nurse signature and .....not in that 3rd shift nurses handwriting....but the writing belonged to the day nurse who keeps time switching....mind you....there is no signature following the "refused meds" bit either and this was when i noticed the time change. so.......at this point im reallllly sick of this nurses action and changing the med times around 1 without telling me and 2 without regard to the fact that this pt gets so many other bp meds @ hs that could be dangerous.......so I go get on the phone with the doc and asked him about it.....he said and I quote " o hell no toots.....that drug is supposed to be 8 hrs apart if its tid and 12 hrs if its bid "....where from there i wrote out a clarification order including the exact times 12 4 8.....and i charted this. Anyway.....I think that this nurse is out to just spite me or push my buttons....she gives me a weird vibe almost every day....one day shes ok....the next shes on a rampage and starts acting like a maniac....i never know how to take her....when i did what the don told me to do one day....omg......she went slam off on me for trying to explain to her what the don wanted done......she argued and argued and said "Well....I know where EVERYTHING goes " etc etc...and flung her hands up..so i just left it at that and said "glad to know you know it all" and went on my way. I dont know if shes got trouble at home, with money, or what...I dont know and I would never ask...not my business but I only get this vibe from her..no one else and im not the only one to notice it. the cnas cannot stand her...on days or evening shift...she can be really mean and rude to them...its to the point that my stomach knots up if i see thats shes working the hall I work and i hate getting report from her....she used to be friendly and funny and really nice.....honestly i think she just doesnt like me since i tried to explain the new charting info that was given to me and other nurses by the boss.....what id like to know is .....1 how many spiteful nurses have you guys had the joy of working with and how did you handle them without drama.....2. do you still work with them? 3. what things have other nurses done to you or other coworkers that were either unsafe, unfair,unethical, or just to get someone else in trouble......i wouldnt dare bring this issue up with any of my coworkers ....things might get misconstrued or twisted around and lead to drama that i want no part of. my belly cant take much more of this chick before it explodes! thanks so much :)

I am sorry you are having difficulty with this co=worker. In the hospital we also have to readjust medication times for our dialysis patients. No sense giving a medication which will be washed out by dialysis. Harder still somedays we do not even know whether a patient will be going in the morning or late afternoon, now that makes it one heck of a guessing game. I have no great ideas for this, we simply do the best we can.

this one goes at 11am m w f ....but this isnt the first time this nurse has pulled this stunt. We have a really neat machine at work that looks like a huge glucometer but it tests pt/inr's ... what is supposed to happen is when you get the result its wrote on a flow sheet for each pt + on the mar .....this hasnt been getting done and just so happens...with this exact same pt who is on coumadin....their flow sheet disappeared and no results were on the mar so who got chewed for it? me. day nurses do the pt/inr's and the boss came down on me about it.....and told me to make sure the other ones knew to write it down on the mar....I told her i wouldnt do that b/c shes the boss, not me and if they dont listen to her theres no way anyone would listen to me. I told her about this problem about 8 mos ago and she said shed address it and never did. when they first started using this machine everybody was good about writing it down in the right places but then got slack and stayed that way. to me it strikes me as just sloppy nursing. so many nurses hate the boss that they just do whatever they want and disregard her instructions.....i always try to do what she says to do if its within reason and my scope and its ethical.....i think this is why she expects me to be able to clean up the messes thats left behind by coworkers. Everybody's got a boss somewhere in the workplace.....so I like to keep life simple and drama free. What Im afraid of is coming to blows with this nurse one day. or maybe im just overly annoyed by a know it all who doesnt know as much as she thinks she does. i am just so confused!

Dialysis often causes a significant drop in BP anyways so their BP med needs may be labile. If I were in your shoes I would discuss this with your facility pharmacist.
