Standard to metric tables

Our hospital is converting our system from standard to metric. I have been scouring the internet and this forum for printable medical metric equivalencies reference tables and I'm not able to find anything beyond the mathematical formulas, cooking measurements, or samples of conversions that aren't in real world ranges. What I'm looking for is a table that I can print and have posted in ranges for normal adults that has a list of weights in pounds then kilograms, heights in inches then centimeters , and temperature readings in F then C. Where can I find this? Does anyone have computer files of these that I can have and print out?

I just completed an internet search for "imperial to metric conversion chart."  Like yours, most of the results of my search wouldn't be helpful.  

If you're Excel savvy, you should easily be able to create something useful.
