Staff Assessment

Hi everyone!!

Full-disclosure: I am a clinician who inputs into the MDS regularly, but I am not an MDS coordinator.

Quick (and I hope simple) question: the clinicians have been provided a couple conflicting directives on how to complete discharge MDS. When a patient has an unplanned DC and we were unable to interview, we will put "not assessed," unless we know the pt well enough to do a "staff assessment." Are we required to speak with other clinicians (e.g., nursing) to gather this data to fill in our sections OR is it appropriate to do "not assessed" if we, as the clinicians inputting, do not have the information?

For unplanned discharges, interviews are not required or disabled by the software. The staff assessment, however, will need to be completed. All staff members who has direct knowledge of the resident and the family may contribute to the assessment. "Not assessed" only applies if no staff member have had the opportunity to interact with the resident regarding those areas of mental skills or a family member is not available to provide the information.
