Small office vs. Hospital work

:o currently working in a private family practice x 4 years. loved it in the beginning because i was previously working er in california (loved that too) but now i had some established pt contact again rather than in the door-patch 'em up- out the door contact. originally started in family practice then onto d.o./physical therapy, then ob/gyn, urgent care, then back to private practice. but with one dr. and a part time p.a. my 14 year experience in different settings has seem to benefit them greatly as far as being able to "dump" some of the patient care that should be their responsibilty during a pt.'s visit. it's my own fault to an extent for letting this slowly get worse over the past 4 years but doing all rx refills, notify pt's of "bad" test results, starting pt's on new meds and giving consults on diabetes, hyperlipidemia, monitoring pt/inr and changing coumadin dosing i think should be part of the pt's visit with their dr. a few months ago i actually had to show the p.a. how to do a catheter.........i 'm sure we all feel used and abused out there but there are certain things that a private family dr is still responsible for doing/practicing, being lazy and dumping it on his nurse is not one of them. am i bitching about nothing??? makes me want to go back to hospital work where i wasn't the only nurse but rather "hey you" ...........................................
