I'm just starting these, but I find this has been applying to myself lately:
Diagnosis: acute need for chocolate related to moodiness, general craving as evidenced by the pile of wrappers, preoccupation with chocolate.
Intervention: Must make chocolate more available.
Outcome: General satisfaction.
Please, add your own. I would love to read them.

Diagnosis: Risk for insanity R/T information overload from excessive studying AEB increasing stress and anxiety.
Intervention: SPRING BREAK!
Outcome: Sweet, sweet relief!

We had this on the back of or oktoberfest shirts a few years back. It is a huge thing where I went to school
Diagnosis: Stress, sleep deprivation and self care deficit
AEB: Restlessness, anger and lack of patience
Interventions: Beer with supplemental Jager Bombs
Evidenced based Practice: Has been shown that time with friends and alcohol lowers care, stress level and helps one sleep
