Do I need to answer that I already passed the NCLEX-RN or just the CGFNS in my visa screen? if ever, is it alright to just leave the question pertaining to NCLEX blank? cause I was told that if I'm going to include details about passing NCLEX in applying for visa screen certificate they will ask me additional fee for verification.

If you took the NCLEX exam, you are going to need to get verification from the state that you wrote it for. To leave off information like that would be considered fraud, and you could not receive a VSC at all because of it. That is a very important piece of information that needs to be verified.

You do not list the license if you have not had one assigned, but it must be verified that you passed the exam. And yes, you are going to need to pay the fee associated with that. But better to pay it, then get denied for providing a false document to US Immigration and that is what you would be doing.
