Hi,
I basically understand how to create a careplan however I am a bit confused about the rational part. I understand you need the scientific rational behind the intervention, but where are you supposed to get this from? Just search through articles or textbooks?
Im currently using the "Nurses Pocket Guide, Diagnoses, Interventions, and Rationales". However, there are only rationales for some of the interventions.

I use Nursing Diagnosis Handbook: An Evidence-Based Guide to Planning Care by Ackley & Ladwig (8th Ed.).
It has rationales for every intervention.

Agreed with the previous posters, used the Nursing Diagnosis Handbook. It gave rationales for everything. If you can't afford it I have a version I could send you.
